One of the reasons why problems such as you mention in the opening post exist is that all these are associated with your natural tendencies, the way you are and the way you have been since birth. Most people make terrible traders simply because their psychological frailties, which are seldom exposed in ordinary day to day life, are ruthlessly and unconditionally exposed in a trading situation in which cool, decisive decisions have to be made. All successful traders have confronted and overcome these weaknesses, usually after many years of pain and frustration, and they have learned to become and think like traders have to because it is the only road to success.
This means that if you are sabotaging your trading the way most traders do, then you have to shed the skin of fallibility we all start out with, and become the person you need to be to trade the markets. This does not come easily or quickly however, you cannot for example tell yourself in the evening that tomorrow you will be more disciplined and expect it to happen, because it won't. Essentially, your whole genetic make up has to be re-wired to think like a trader thinks, and it is a process. However, so long as you realize this, and keep working hard towards it, it will come. Speak to any successful trader, and I guarantee they will say more or less the same thing.
