I downloaded the indicator and attached to chart but when I pull up MT4 editor I cannot see any place where I can input my email address? I am assuming of course I need to modify it so that I receive an email alert?

Email address is in your preferences for metatrader. Go to menu, tools, options, select the "email" tab and insert all your information there.

Or even better, before you do that, go to www.metaquotes.net website and send them an email reporting the problem. They're very quick with replies. Usually same day.Ignored

No luck... Not sure exactly I am doing the right thing but I know the smtp for my email account and the ip address but I still cannot send email when I click on " Test"
If I am understanding you correctly I should type in the smtp of my server followed by 2525?
In addition to this I need to enter my password for my email address account?

I don't think you need 2525. It's something i got from my support, don't even know what it is.
Remember to check Enable
Use your SMTP from your account I don't think you gonna need your IP.
SMTP login and password is the same you use to login to your mail account.
Mabye you also need to fill out the the field with from and to
