Trading Diary/Journal

Ok, I'm still in the reading books stage(past two months) of my forex education. I'm now ready to start doing Demo Trading, but I want to create a simple trading Journal or Diary--because I hear that it's paramount to my success. But none of the books I have, have shown me a Template or example for a good trading Diary/journal. Can someone plz show me a good example of what a forex trading diary should look like? plz this is very important.

I write down the pair traded, entry price, stop, reason for entry (TA) and exit result (profit or loss). I also make notes about what happened after I exited the trade. I learned I got stopped out frequently by a few pips and then I would have had a profitable trade. I now know I need to be patient and get better entries or use bigger stops. I also realize that I have entered trades for no reason other then it looks like it is moving and I want to jump on. Those (emotional ) trades always end up as losers. Like Nike, "just do it". This has really helped me learn about my errors and hopfully not repeating them.

Thanks for replies guys. One more question, should I have 4 record keeping tools such as suggested in "Come into my trading room"? Dr. elder is talking about trading everything but Forex, so would these 4 record keeping ideas b helpful in Forex also?

Are you more of a visual learner? Why not print a screen shot of your trade and write or draw the info on it that you would like. Maybe not as organized as Norbecker's form but you can see the whole picture at once. If you are not sure yet what info is important use the form as a guide.
